How to Microdose Semaglutide: The Steps and the Two Traps

This page is the semaglutide-specific layer, because two of the biggest hazards in this practice are things that are true of semaglutide and not of tirzepatide: what is actually in a compounded semaglutide vial, and how easy the arithmetic is to get catastrophically wrong. Both are documented by FDA, not inferred by us.

Where a microdose sits on the semaglutide ladder

Semaglutide's approved doses start at 0.25 mg once weekly, and that rung is a tolerance-building step rather than a maintenance dose — the labels escalate away from it deliberately 1. A microdose is anything below that step, which places it beneath every dose any label defines and beneath the lowest dose the dose-finding trials studied.

That last point is worth sitting with, because it sets the ceiling on what any technique below can deliver. In the phase 2 dose-ranging trial, semaglutide's weight effect fell steadily as the dose fell: roughly 6% at 0.05 mg per day against about 14% at the top dose tested 2. Doing the procedure perfectly does not move you onto that curve. It places you below where the curve was drawn.

Trap one: the salt-form problem, in FDA's words

This is the semaglutide-specific hazard with no tirzepatide equivalent, and it is not a rumor.

FDA states plainly that "salt forms should not be used to compound semaglutide", that it is aware some compounded semaglutide products are salt forms, and that these — semaglutide sodium and semaglutide acetate — are "different active ingredients than are used in the approved drugs." The agency adds that it does not have information on whether these salts share the chemical and pharmacologic properties of the approved active ingredient 3.

Read that carefully, because it changes what a dose calculation even means. If the vial contains a salt form, you are not measuring a smaller amount of the drug studied in the trials — you are measuring a different substance whose potency relative to the approved one is not established. Every milligram figure downstream of that is a figure about something else.

So the first practical step in microdosing semaglutide is not arithmetic. It is asking the pharmacy, in writing, which form of semaglutide is in the vial — the base, or a salt — and declining the product if the answer is a salt or is not forthcoming. Trap two: the arithmetic that hospitalizes people

FDA reports "multiple reports of adverse events, some requiring hospitalization, that may be related to dosing errors associated with compounded injectable semaglutide products," and is explicit about where the errors came from: "patients measuring and self-administering incorrect doses of the drug, and in some cases, health care professionals miscalculating doses" 3.

Note the second half of that sentence. These are not only patient errors — clinicians miscalculated too. The reason is structural, and it is specific to how compounded semaglutide is sold: there is no standard concentration. A branded pen has a fixed, labeled strength. A compounded vial's concentration is whatever the compounder chose, which means the same "0.25 mg" is a different mark on the syringe from one vial to the next, and a vial refill from a different batch or pharmacy can silently change it.

The arithmetic itself is simple, and our microdose calculator runs it: volume in milliliters equals dose in milligrams divided by concentration in milligrams per milliliter, and units on a U-100 insulin syringe equal milliliters times 100. What makes it dangerous is not the formula but the input. Get the concentration wrong by a factor of ten — an easy slip when a vial is labeled in total milligrams rather than milligrams per milliliter — and you have a tenfold dosing error in a drug where the therapeutic ladder spans a factor of ten from top to bottom.

Two habits reduce that risk more than any chart:

  • Recompute from the vial in front of you, every single time, including on refills. Never carry a unit number across vials.
  • Read the label for the units it is actually stated in. "5 mg" on a vial is a total quantity, not a concentration. It becomes a concentration only once you know the volume it is dissolved in — which for a lyophilized powder is the volume you added.

The branded pens are not a workaround

People sometimes reach for a branded pen to escape the concentration problem. It does not work, for reasons covered in how many units is a microdose in an Ozempic or Wegovy pen?: most Wegovy pens are single-dose fixed injectors with nothing to dial, and the Ozempic pen's dose window shows approved doses rather than units, so hitting a sub-dose means stopping between marks and estimating. The pen removes the concentration uncertainty and replaces it with device-manipulation uncertainty. Neither is measurement.

What "start low and hold" actually means here

If a clinician has agreed to supervise a low semaglutide dose, the cadence question people ask most is whether they must climb. The labeled schedules escalate on a fixed timetable toward a maintenance dose 1, and choosing to hold below the first rung is a departure from that schedule, not a slower version of it. One thing worth saying plainly: semaglutide's long half-life means dose changes take weeks to fully express. Judging a low dose after one or two injections is judging an incomplete picture, which is a common way people talk themselves into escalating faster than they intended.

The honest bottom line

The procedure is not the hard part. Confirming you have the right substance is, and FDA has told you what to ask: not a salt form 3. Confirming the concentration is the other hard part, and FDA has told you what happens when it goes wrong — dosing errors serious enough to hospitalize people, made by patients and clinicians alike 3. A pharmacovigilance analysis of compounded GLP-1 receptor agonists found the same pattern from the other direction: markedly elevated reporting odds for preparation errors, contamination and compounding problems 4.

And when all of that is done correctly, the dose-response reality is unchanged — less drug did less in the trial that measured it 2. Do this with a clinician who knows you are doing it, and ask the pharmacy the salt question before the first injection.
