This page is the tirzepatide layer, and it exists because tirzepatide's practical problems are not semaglutide's. Where microdosing semaglutide turns on which substance is in the vial, tirzepatide turns on something more unsettling: FDA has found compounded tirzepatide whose label names a pharmacy that does not exist.

That single fact reorders the steps. The usual advice — read the concentration off your own vial and recompute every time — assumes the label is telling you the truth. Sometimes it is not.

The floor you are dosing below

Tirzepatide's approved ladder starts at 2.5 mg once weekly, and that rung is a tolerance-building step, not a maintenance dose — the label escalates away from it on a schedule 1. A microdose sits below it, outside anything a label defines.

What that costs you is measurable. In SURMOUNT-1, the phase 3 obesity trial, tirzepatide's weight reduction was substantially greater at its higher maintenance doses than at its lowest across 72 weeks 2. Step 1 — Establish that the vial is what it says it is

FDA reports that it "is aware of fraudulent compounded semaglutide and tirzepatide marketed in the U.S. that contain false information on the product label." It is specific about the forms this takes: in some cases "the compounding pharmacies identified on the labels of the products do not exist", and in others the label carries the name of a real licensed pharmacy that "did not compound these products." FDA notes one reported adverse event tied to a product labeled as compounded tirzepatide from a pharmacy that had not made it — redness, swelling, pain and a lump at the injection site 3.

This is why "read the label carefully" is not, by itself, a safety step. A concentration printed by someone willing to invent a pharmacy is not a number you can do arithmetic with.

The practical response is to verify the chain rather than the print:

  • Get the pharmacy's name from the prescriber, not only from the vial, and check it is a real, state-licensed pharmacy that will confirm it filled your prescription.
  • Prefer providers that name their compounding pharmacy publicly before you buy. Most do not — it is rare enough that we treat it as a distinguishing mark in our provider rankings, and it is the single easiest thing to check in advance.
  • Treat an unnamed pharmacy as a missing input, not a neutral detail.

Step 2 — Find your real concentration, which depends on the presentation

Tirzepatide has a wrinkle branded semaglutide does not: even Lilly's own product uses two different volumes per dose across its four presentations. Single-dose pens and single-dose vials deliver a dose in 0.5 mL; the multi-dose vial and the single-patient-use KwikPen deliver it in 0.6 mL 1. Same milligrams, different concentration, different number on a syringe — 2.5 mg reads 50 units from one and 60 from the other.

The rule that matters here is that a unit figure belongs to one container. It does not transfer between presentations, between compounders, or across a refill.

Step 3 — Start low and hold, and understand which direction the reported harm runs

Here is a distinction most microdosing content skips. FDA's adverse-event reports on compounded tirzepatide and semaglutide describe patients dosed "beyond what is in the FDA-approved drug label" — using more product in a single dose, dosing more frequently, or increasing the amount more quickly than the labeled titration 3. The documented pattern of harm runs toward too much, too fast, not too little.

That cuts both ways for anyone microdosing. It is reassuring in that the reported hospitalizations are not stories about people taking too little. It is a warning in that the most common way a microdosing plan goes wrong is abandoning it upward — getting impatient at four weeks and escalating faster than any label contemplates. FDA explicitly asks prescribers to be vigilant about titration schedules for compounded products 3.

What this page deliberately does not claim

FDA's salt-form warning — that semaglutide sodium and semaglutide acetate are different active ingredients and should not be used to compound semaglutide — is written about semaglutide 4. We are not going to translate that silence into a clean bill of health for compounded tirzepatide. What can be said accurately is narrower and more useful: FDA has published a specific salt-form instruction for one molecule and not the other, so the salt question is a documented issue you can ask about on the semaglutide side, while on the tirzepatide side the documented issues are label fraud and dose-beyond-label reports 3.

The general compounding risk applies to both. The honest bottom line

Microdosing tirzepatide is not primarily a measurement problem, because the measurement is easy once you know the concentration. It is a provenance problem. FDA has documented labels naming pharmacies that do not exist 3, and no amount of careful syringe work rescues a number printed by someone who invented the source. Establish who compounded it before you calculate anything, recompute from the container in front of you rather than a chart, and expect the pressure on your plan to come from the direction FDA's reports actually point — toward escalating too fast.
