In one paragraph
The verdict
Nuform is a physician-owned clinic in Michigan with a more personal setup than most online sellers: a dedicated provider and a specialist you can call or text, and a check-in every four weeks. Its weight-loss products are sold under the names GLP Vitamin Blend and GLP+GIP Vitamin Blend; you have to read the FAQ and a site banner to learn that it offers semaglutide, including an oral form, and tirzepatide. Every price is a prepaid kit, printed with its length, and the per-month figures beside them are those totals divided out. There is no month-to-month option. The starter kits include dose increases at one price, which suits someone titrating up and buys nothing for someone staying small. A physician fee is charged on top, and the site never says how much it is. The kit lengths in the FAQ do not match the ones on the price cards, and the pharmacy class is given two different ways. No bloodwork is needed to start, and medication orders are final once placed.
Microdosing is off-label and unproven — decide with a clinician. Prices are as published by the provider when we last checked; confirm current terms before you buy.
Who it suits, and who it does not
A good fit if
- You want a named care team you can phone or text
- You are ready to commit to a few months of treatment up front
- You want an oral semaglutide option
Look elsewhere if
- You want to pay month to month
- You want the drug named on the product you buy
- You want to know the full cost, physician fee included, before you start

- Standard plan, self-directed low dose
No low-dose product of its own. Starting low means asking for a smaller amount inside a standard plan, at the standard plan's price.
235 of the 548 programs here price a small dose this way. Sold only as prepaid kits of two and a half to six and a half months, with no month-to-month rung, and the product cards name the drug only as a vitamin blend. Starter kits include dose increases at one price, and no low-dose product exists.

- Oral semaglutide is offered as well as injections

How the program works
Nuform Health and Wellness, LLC is a physician-owned clinic in Troy, Michigan, founded by a D.O. and a physician assistant. For weight loss it sells two compounded products named only GLP Vitamin Blend and GLP+GIP Vitamin Blend; elsewhere the site says it offers oral and injectable semaglutide and names tirzepatide in a banner. Each comes as a starter kit that includes dose increases and a longer maintenance kit, all paid as a prepaid block. You get a dedicated provider and a specialist you can call or text, with follow-ups every four weeks, and no bloodwork is required for GLP-1 treatment. Medication comes from U.S. compounding pharmacies, described once as 503A and B and once as 503A only, and none is named. Medication orders are final once placed with the pharmacy; the physician fee is refundable until the consultation begins. The states served are not listed. Nothing describes a low-dose program.
The honest cons
There is no month-to-month option: the cheapest way in is a prepaid kit of two and a half or three months
- The product names hide the drug; neither card says semaglutide or tirzepatide
- A physician fee sits on top of the kit price, and its amount is not printed
- The FAQ says kits are three or four months, while the price cards run from two and a half to six and a half
- Medication orders are final once placed, the pharmacy class is given two ways, and no pharmacy is named
- Compounded GLP-1s are not FDA-approved, and intentional microdosing is off-label and unproven

Intentional GLP-1 microdosing is off-label and unproven — no randomized trial has established that a sub-label dose does anything, and any dose belongs with a clinician who knows your history. If you are going to have that conversation, have it with real numbers.
